A Tribute Gift is a meaningful way to let others know you care, while helping advance the mission of Proctor Hospital.

Tribute gifts can be made for a variety of occasions, such as

• Weddings
• Graduations
• Thinking of You
• Anniversaries
• Holidays
• Special Achievement
• Promotions, job changes, or retirement
• Other events and occasions

An acknowledgement of gift will be sent to the person being honored, without reference to the size of the gift. You will receive a copy of the notification along with an acknowledgement of your gift for tax purposes.

A Memorial Gift will honor and celebrate the life of a friend, colleague or loved one in a dignified way that will help others.

If you would like to request memorial gifts be made to the Proctor Health Care Foundation in honor of a loved one, please ask the funeral director to insert the following in the obituary notice:

The family suggests that memorial contributions be made to the Proctor Health Care Foundation, 5409 N. Knoxville Avenue, Peoria, Illinois 61614.

Each gift will be acknowledged to the donor for tax purposes, and to the family of the person being memorialized without reference to the size of the gift.
